New Health Consulting – Expert Nurse-Led Comprehensive Geriatric Assessment
At New Health Consulting, we provide in-depth, person-centred Comprehensive Geriatric Assessments (CGA) to ensure older adults receive the right support for their medical, functional and social needs.
We come to your preferred setting—whether at home, in a care facility, or another location—to conduct a detailed, face-to-face assessment.
Each CGA includes:
✔ A holistic evaluation of physical health, mobility, cognition and social well-being.
✔ A tailored, professional report with clear recommendations for care, support and interventions.
✔ An optional Structured Medication Review (SMR), focusing on safe, effective prescribing.
A full CGA typically takes 3–4 hours, ensuring a thorough and accurate assessment. If an SMR is required separately, this requires approximately 1 hour.
All of our services include a detailed, patient specific report covering all domains of the chosen assessment with onward recommendations for the patients GP, healthcare provider and more.
We primarily cover North East England, but can travel nationwide by prior arrangement.
The British Geriatrics Society (BGS) defines a Comprehensive Geriatric Assessment (CGA) as a multidimensional, interdisciplinary process aimed at optimising the medical, functional, psychological and social needs of older adults. CGA involves:
Medical assessment – Identifying and managing comorbidities, polypharmacy and frailty.
Functional assessment – Evaluating mobility, ADLs and risk of falls.
Psychosocial assessment – Assessing cognition, mood and social determinants of health.
Environmental assessment – Reviewing home safety and care support needs.
Care planning – Developing a coordinated, person-centred management plan with input from a multidisciplinary team.
CGA's have been shown to improve clinical outcomes, enhance functional independence and reduce hospital admissions in both acute and community settings.
A CHC (Continuing Healthcare) assessment determines eligibility for NHS-funded ongoing care for individuals with complex health needs. The process includes:
A CHC is particularly relevant for frail individuals with progressive or complex care needs and a CGA can provide supporting evidence.
